The Chronic disease management approach can be applied wherever guidelines have been developed to prevent complications of an illness.
Failure to manage chronic disease foreseeably leads to complications occurring earlier than would have been expected where there is evidence of the effectiveness of the interventions.
Cardiovascular disease models are sufficiently accurate to estimate how long this interval would have been with reasonable management giving a result that can be measured in weeks.
Other chronic diseases are developing evidence bases that can predict the effectiveness of the various components of the management plan.
